Letter Request Form

This form is for students who need general information from the office in order to fulfill questions from employers, external offices, landlords, OPT, etc. Please follow the form link for additional information about letter types.

Letter of Completion Form

This form is for students who are applied for an upcoming graduation, and have already met all the requirements for their degree. Since we cannot post degrees before graduation, the records office can write a letter verifying completion of the degree. Please print and complete this form, and schedule an appointment with a Graduation Specialist.
